Forms & Permits
All information fields on each applicable form must be completed as required.
Incomplete information and/or illegible handwriting will require you to fix the deficiencies and re-submit the form. This will delay processing of your request.
Completed forms are to be submitted to our BPGCD District office either by:
- US postal mail
- hand delivery
- or by email per our Contact page on this website.
If payment is required, you must include on your check sufficient identification to allow us to match your payment with your request (as applicable, include your phone number, your personal name, your company name, well number, well address, date of form submission, etc.).
We suggest you use a letter-size manilla envelope to secure your documents and check as a single package. Use a separate envelope for each well. Label your envelope just as you label your check. We only accept cash or checks, not debit/credit cards.
